Due to the retirement of the current post holder an exciting opportunity has arisen for a Team Lead Adult Social Care within the Integrated Discharge Team at Salford Royal Hospital Foundation Trust.

The Hospital Integrated Discharge team is a seven day service comprising of Social Workers, Advanced practitioner Social Work, Community Assessment Officer, Patient Pathway Managers, Nurse Advanced Practitioners and a Housing Officer.

The team supports the ward to facilitate safe and timely discharges from the hospital, reducing length of stay and delayed transfers of care. To achieve this they use a wide variety of personalised support, assisting people to promote their independence, supporting people with complex situations to ensure they have the maximum level of choice and control available to them promoting the principles of The Care Act and MCA.

You will manage the social work practitioners within the Integrated Discharge Team and be expected to work alongside the Team lead for health ensuring people flow through the hospital, facilitating safe and timely discharges.

We continually strive for improvement and it is expected that you will work with the Quality Improvement team in driving forward change for the good of the service.

You will be supported throughout your time with us by dedicated teams from Practice Development, Leadership and Learning Development. This support includes access to specific programmes, e-learning and bespoke CPD funding to help you access university modules linked to your goals and objectives to take your career in the right direction. The organisation boasts state of the art facilities to support your education including seminar rooms, IT suite and a trust library.

You will be employed by Salford Integrated Care Organisation. Continuous previous service can be considered at the discretion of the Trust. Continuous service with a non-NHS organisation, such as a local authority, may be recognised for annual leave and sick-pay entitlement and pension scheme. The Trust will seek evidence of continuous service .
