A temporary vacancy has arisen for a Social Worker to be based at Cromwell House Adult Community Mental Health Team. Salford Mental Health Services are committed to providing high quality, integrated services where the focus is upon recovery and self-directed support.

Social Worker HCPC registration is required.

You will be a qualified Social Worker (AMHP is desirable, not essential). We are looking for an experienced mental health practitioner who has high professional standards and the ability to work well with colleagues in a multi disciplinary setting.

You will have a keen focus on promoting the independence and promote the best possible outcomes for people with mental health needs.

You will be able to work within a budgetary framework and demonstrate your ability to initiate new approaches to service delivery.

Our Community, Health and Social Care directorate has great partnerships with the local CCG, NHS trusts and third sector. In fact we are one of NHS England’s national Vanguards for integrated care and we are creating an integrated care organisation that will bring together all health and social care professionals across the city.

If this is approved in April 2016, it is proposed that the service transfers to the Integrated Care Organisation from July 2016 under the employment of Salford Royal Hospitals NHS Foundation Trust. Therefore you should note that any new appointments starting after 1st July 2016 may be made on NHS Agenda for Change salary and terms and conditions.

You must be willing to work flexibly in our organisation.

You will be based at an adult community mental health team within Greater Manchester Mental Health NHS Foundation Trust. The post holder will provide assessment for service users and carers, in line with the Care Act (2014). The post holder will act as a care co-ordinator in line with the Care Programme Approach (CPA).

You will be an Approved Mental Health Professional (AMHP) or willing to undertake the relevant training, and will carry out the statutory duties of an AMHP in accordance with the relevant legislation and policies.
 
You will work to the HCPC Standards of Proficiency for Social Workers in England.
 
You will report to the Team Manager for day to day management. Professional social work supervision will be provided by an Advanced Social Work Practitioner.

You will aim at all times to promote the independence and well-being of adults with mental health needs; assist social inclusion and valued community presence and enable the widest possible expression of chosen lifestyle.
 
The post is permanent, Monday to Friday with Acute Care Pathway working one day per weekend in 12.
 
In return we offer regular supervision from experienced management teams, working within an integrated health and social care team, and plenty of opportunities to be involved in the changing world of Adult Social Care.
 
Salford Royal has teamed up with housing association Salix Homes to provide a 26-apartment complex for doctors, nurses and other clinical staff new to the Trust. Please enquire upon appointment to review availability.
